Clixlogix-built AI Care Companion for cardiovascular care company on Microsoft Azure
Clixlogix delivered an eleven month engagement for a Massachusetts cardiovascular care company spanning strategic assessment, discovery, solutioning, and phased implementation. The solution replaced a vendor-locked patient application with a custom Flutter platform on existing EHR REST APIs, added an AI care companion grounded in the client’s clinical protocol using Azure OpenAI Service and Azure Health Data Services managed FHIR API, and introduced AI session intelligence, enrollment automation, billing automation, RPM anomaly detection, computer-vision lifestyle logging, and standardized partner reporting.

- Patient attrition and revenue leakage in the referral-to-billing funnel.
- Low self-scheduling adoption, PBM enrollment failures, manual RPM review, fragmented partner reporting, and a vendor-locked patient app.
- Unused session transcripts that were not yet processed for protocol adherence or compliance monitoring.
- Built a custom Flutter patient app and a React B2B partner portal on top of intact EHR data via REST APIs.
- Implemented an Azure OpenAI Service-based AI care companion using retrieval augmented generation grounded in the client’s clinical protocol and patient health data.
- Added rolling-transcript session intelligence for real-time protocol suggestions and post-session compliance reports, direct PBM enrollment automation for primary carriers, an AI billing agent, RPM anomaly detection, computer-vision lifestyle logging, telehealth via WebRTC, and standardized partner reporting.
- Within 90 days, monthly patient attrition fell by approximately one fifth.
- Daily active app users tripled compared with the previous vendor portal baseline.
- The AI care companion handled over three fifths of routine patient queries.
- The median referral-to-active-patient window dropped to under 48 hours.
- Hospital partner reporting became fully automated, and post-session compliance monitoring shifted from manual review to automated transcript analysis.
Architecture
A custom Flutter mobile application and React partner portal were built on existing EHR REST APIs. Microsoft Azure hosted the solution using Azure OpenAI Service for the AI care companion and session intelligence, Azure Health Data Services managed FHIR API for clinical data integration, encryption at rest and in transit, audit logging, and role-based access control. AI session intelligence used a rolling transcript pipeline. The implementation also integrated Salesforce Health Cloud, Waystar, Twilio, and WebRTC.
